Documents Needed for Change in Employer - Single Permit

For CAB or Taxi Drivers

Valid Residence Card: Ensure it is current and unexpired.
Valid Passport: Needs at least 6 months of validity.
Lease Agreement: Must be notarized.
Lease Attestation Form: Also needs notarization. click here
Declaration by Landlord: Complete the C3/C3.1 form. click here
Housing Authority: Relevant documentation as required.
Landlord ID Card: Include their signature on it.
Medical Report: Not required if the applicant has been in Malta for the last 3 years, but provide the last available report for reference.
Maltese Driving License: A valid license is mandatory.
Transport Malta TAG: Necessary for work-related driving.
Signed CV: Highlighting driving experience either from Malta or the country of origin.
Termination Letter: Or employment history from Jobsplus.
